Remove redundant includes. 
"fluid_synth.h" is already included by "fluid_lash.h".
"fluidsynth_priv.h" already includes all these files, with the conditional inclusion.

Add conditional inclusion for signal.h 
HAVE_SIGNAL_H macro is already handled by CMAKE, but signal.h is not included here (it's required for LASH).
